Job Summary
To provide coordinated care services for injured workers with low to moderate complexity claims, the full-time Care Coordinator Specialist will manage medical care needs, facilitate communication among stakeholders, and support return-to-work efforts under RN supervision.
Key Responsibilities
- Coordinate routine medical care and facilitate communication between injured workers, providers, and employers
- Monitor adherence to treatment plans and maintain accurate documentation of medical history and case progress
- Schedule medical appointments and ensure cost containment through the utilization of in-network providers
Required Qualifications
- High School Diploma and graduate of an accredited practical/vocational nursing program
- Current compact LPN/LVN license in good standing
- 2+ years of clinical experience, with preference for workers' compensation or disability management experience
- Experience in healthcare coordination and documentation
- Proficiency in Microsoft Office Suite and basic data entry skills
