Compact LPN Care Coordinator

Job Summary

To support injured workers with low to moderate complexity claims, the full-time Care Coordinator Specialist will manage care coordination, facilitate communication among stakeholders, and ensure continuity of medical services under RN supervision.

Key Responsibilities

• Coordinate routine medical care needs and facilitate communication between injured workers, providers, employers, and adjusters
• Monitor adherence to treatment plans and maintain accurate documentation of medical history and case progress
• Support return-to-work efforts and ensure cost containment through the utilization of in-network providers

Required Qualifications

• High School Diploma and graduate of an accredited practical/vocational nursing program
• Current compact LPN/LVN license in good standing
• 2+ years of clinical experience, with preference for workers' compensation or disability management experience
• Experience in healthcare coordination and documentation preferred
• Proficiency in Microsoft Office Suite and case management systems
