Compact LPN Care Coordinator

Job Summary

Working under RN supervision, the full-time Care Coordinator Specialist will provide coordinated care services for injured workers with low to moderate complexity claims, ensuring continuity of medical services and facilitating communication among all parties involved.

Key Responsibilities

• Coordinate routine medical care needs and facilitate communication between injured workers, providers, employers, and adjusters
• Monitor adherence to treatment plans, maintain documentation, and facilitate return-to-work efforts
• Perform administrative functions including case documentation, time management tracking, and attending team meetings

Required Qualifications

• High School Diploma required; graduate of an accredited practical/vocational nursing program
• Current compact LPN/LVN license in good standing
• 2+ years of clinical experience required; workers' compensation or disability management experience preferred
• Demonstrated organizational and technological skills, including proficiency in Microsoft Office Suite
• Basic understanding of disability management principles
