Compact LPN Clinical Liaison

Job Summary

As a critical member of the clinical care team, the remote Compact LPN Clinical Liaison will provide clinical and administrative support to ensure high-quality member care, including documenting patient encounters, coordinating care activities, and maintaining accurate medical records.

Key responsibilities

• Document patient/member encounters and maintain complete medical records in Thyme Box and supported EHRs
• Coordinate care activities, including referrals and follow-up appointments, to ensure continuity of care
• Monitor clinical communications and triage messages to appropriate providers and departments

Required qualifications

• At least 2 years of experience as a Licensed Practical Nurse (LPN), preferably with telehealth experience
• Possession of a compact LPN license
• Experience with electronic health records and comfort using new technologies
• Strong organizational skills and ability to manage multiple tasks under pressure
• Effective listening and communication skills to build rapport with members and colleagues
