Job Summary

A company is looking for a Proactive Titration Licensed Practical Nurse for weekend coverage.

Key Responsibilities

• Contact patients to discuss medication titration recommendations and ensure adherence to treatment plans
• Maintain detailed documentation of patient interactions in compliance with HIPAA and clinical protocols
• Provide timely and empathetic communication with patients and healthcare providers

Required Qualifications

• Active compact multi-state LPN License
• 5+ years of clinical experience in acute or ambulatory care, preferably in cardiology or nephrology
• Experience in maintaining HIPAA-compliant environments
• Must have a HIPAA-compliant home office setup with reliable internet
• Strong track record of working independently and managing time effectively
