Job Summary
Providing telephonic coaching and educational resources, the full-time salaried Disease Management Care Coach - Cardiology will support individuals with chronic health conditions by assessing needs, coordinating care, and promoting wellness in a 100% remote environment.
Key responsibilities
- Deliver telephonic coaching and referral services to participants managing chronic health conditions with clinical oversight
- Assess participant needs and communicate with healthcare professionals for effective care coordination
- Document coaching sessions and participant activities in the case tracking software
Required qualifications
- Nursing Degree from an accredited college or university
- Active and unrestricted Compact Registered Nurse License
- 2+ years of experience in direct participant coaching
- Intermediate knowledge of Microsoft Office, including Word, Excel, and PowerPoint
- Experience with information database systems for reviewing medical history and data
