Compact RN Behavioral Health Coach

Job Summary

Providing telephonic coaching and educational resources to individuals with chronic health conditions, the full-time Compact RN Behavioral Health Coach will assess participant needs, coordinate care, and promote wellness in a 100% remote environment.

Key responsibilities

• Deliver telephonic coaching and referral services to program participants managing chronic health conditions
• Assess participant needs and communicate with healthcare professionals for effective care coordination
• Document participant activities and coaching sessions in the case tracking software

Required qualifications

• Nursing Degree from an accredited college or university
• Active and unrestricted Compact Registered Nurse License
• 2+ years of experience in direct participant coaching
• Intermediate knowledge of Microsoft Office and database software
