Job Summary

To support a growing team of elite nurses, the remote Compact RN Care Coach will manage a panel of over 100 Medicare patients with chronic conditions, conducting monthly clinical calls to drive outcomes and reduce hospitalizations in a performance-driven environment.

Key responsibilities

• Run monthly clinical calls with chronic care patients
• Drive behavior change through coaching and education
• Maintain precise, compliant documentation using the designated platform

Required qualifications

• Current unrestricted Compact RN license
• 3+ years of clinical experience
• Strong internet connection and dedicated workspace
• Comfortable with multiple software platforms
• Experience in case management or chronic disease management preferred
