Compact RN Care Coach

Job Summary

To support a growing team of elite nurses, the remote Compact RN Care Coach will manage a large panel of Medicare patients with chronic conditions, delivering monthly clinical calls and driving behavior change to improve health outcomes.

Key responsibilities

• Run monthly clinical calls with chronic care patients
• Drive behavior change through coaching and education
• Maintain precise, compliant documentation and update care plans

Required qualifications

• Current unrestricted Compact RN license
• 3+ years of clinical experience
• Comfortable managing large caseloads (over 70 patients/month)
• Strong internet connection and dedicated workspace
• Experience with case management or chronic disease management preferred
