Compact RN Case Manager

Job Summary

A company is looking for an RN Case Manager, Complex Pediatric Specialty - Remote.

Key Responsibilities

• Conduct telephonic assessments to understand patients' medical and social needs
• Develop individualized care plans and provide education on chronic disease management
• Monitor patient progress and ensure continuity of care through proactive outreach

Required Qualifications

• Active Compact Registered Nurse license in good standing
• 3+ years of RN Case Management experience, specifically with complex pediatric cases
• High comfort level with technology, including EMRs and telecommunication tools
• Understanding of accreditation and quality standards (NCQA, HEDIS)
• Ability to work independently and manage multiple priorities remotely
