Job Summary

A company is looking for a Remote RN Case Manager.

Key Responsibilities

• Conducting telephonic assessments to understand patients' medical and social needs
• Developing individualized care plans and providing education on chronic disease management
• Monitoring patient progress and ensuring continuity of care through proactive outreach

Required Qualifications and Education

• Active Compact Registered Nurse license in good standing
• 3+ years of RN Case Management experience, preferably in outpatient or ambulatory care
• High comfort level with technology, including EMRs and telecommunication tools
• Understanding of accreditation and quality standards (NCQA, HEDIS)
• BSN and Case Management certification preferred
