Compact RN Case Manager

Job Summary

A company is looking for a Remote RN Case Manager.

Key Responsibilities

• Conduct telephonic assessments to understand patients' medical and psychosocial needs
• Develop individualized care plans and provide education on chronic disease management
• Monitor patient progress and ensure continuity of care through proactive outreach

Required Qualifications

• Active Compact Registered Nurse license in good standing
• 3+ years of RN Case Management experience, preferably in outpatient or ambulatory care
• High comfort level with technology, including EMRs and care-management systems
• Understanding of accreditation and quality standards (NCQA, HEDIS)
• A quiet, private home office suitable for patient calls and handling PHI
