Compact RN Case Manager

Job Summary

A company is looking for an RN Case Manager for a 100% remote position in the HouseCalls program.

Key Responsibilities:

• Perform telephonic outreach to members for follow-up on referral-related issues
• Ensure members have scheduled appointments with their Primary Care Provider and assist with scheduling as needed
• Provide patient education and connect members to community resources when necessary

Required Qualifications:

• Current, unrestricted Compact RN license in the state of residence
• 3+ years of clinical experience in relevant healthcare settings
• Proficiency in Microsoft Word, Outlook, and Excel
• Ability to work in a production-driven environment
• Must reside in the Central or Eastern time zone
