Compact RN Case Manager

Job Summary

A company is looking for a Case Manager Registered Nurse - Medicare (Remote).

Key Responsibilities

• Facilitate the delivery of healthcare information and benefits to promote member wellness
• Utilize a collaborative process for assessment, planning, care coordination, and advocacy for members' health needs
• Develop and implement health strategies and policies to ensure effective care management

Required Qualifications

• Active and restricted RN Compact licensure
• Minimum 3+ years of clinical practical experience, preferably with Medicare members
• Minimum 2+ years of experience in Case Management, discharge planning, or home health care coordination
• Associates degree or Nursing diploma required; Bachelor's degree preferred
• Willingness to obtain multi-state RN licenses if needed
