Compact RN Case Manager

Job Summary

Facilitating the delivery of healthcare information and benefits, the full-time Compact RN Case Manager will develop and implement health strategies while promoting wellness activities, working remotely and requiring an active RN license in the Central Time Zone.

Key responsibilities

• Facilitate the delivery of appropriate healthcare benefits and information to determine eligibility
• Develop and implement health strategies and programs to promote overall member wellness
• Support network management and clinical coverage policies to ensure timely return to work for members

Required qualifications

• Active and unrestricted Compact Registered Nurse (RN) License in the state of residence
• Located in the Central Time Zone (CST)
• 3+ years of clinical practical experience, preferably in diabetes, CHF, CKD, post-acute care, hospice, or palliative care
• 2+ years of experience in case management, discharge planning, or home health care coordination
• Associate's Degree in Nursing (REQUIRED)
