Compact RN Case Manager

Job Summary

Working remotely on a full-time basis, the Compact RN Case Manager will perform telephonic outreach to members post HouseCalls visits, ensuring they receive necessary education and assistance in scheduling appointments with their Primary Care Providers while adhering to a 9am to 5:30pm EST or CST schedule.

Key responsibilities:

• Conduct telephonic outreach to members for follow-up on referral-related issues identified by Advanced Practice Clinicians
• Assist members in scheduling appointments with Primary Care Providers and refer them to internal departments as needed
• Gather clinical information and provide patient education on disease processes and trigger management during calls

Required qualifications:

• Undergraduate Degree
• Current, unrestricted Compact RN license in the state
• 3+ years of clinical experience as a Registered Nurse in relevant settings
• Ability to work in a call center environment with a dedicated HIPAA-compliant workspace
• Proficient in Microsoft Word, Outlook, and Excel, with solid critical thinking skills
