Compact RN Case Manager

Job Summary

Working remotely on a full-time basis, the Compact RN Case Manager will perform telephonic outreach to members post HouseCalls visit, ensuring they receive necessary education and assistance in scheduling appointments with their Primary Care Provider while managing a variety of clinical needs.

Key responsibilities:

• Conduct telephonic outreach to members for follow-up on referral-related issues
• Assist members in scheduling appointments with Primary Care Providers and refer them to internal departments as needed
• Gather clinical information and provide education on disease processes and trigger management during calls

Required qualifications:

• Undergraduate Degree
• Current, unrestricted Compact RN license
• 3+ years of clinical experience as a Registered Nurse in relevant settings
• Ability to work in a call center environment with a dedicated HIPAA-compliant workspace
• Proficient with Microsoft Word, Outlook, and Excel
