Compact RN Enrollment Specialist

Job Summary

To support patient care initiatives, the full-time remote Compact RN Enrollment Specialist will conduct telephonic enrollment visits, develop care plans, and assist patients in accessing services to improve their quality of life while managing chronic diseases.

Key responsibilities

• Conduct telephonic enrollment visits and develop personalized care plans for patients
• Perform health assessments and collect signed consents for program participation
• Educate patients on medications, medical conditions, and lifestyle choices to promote adherence and positive health outcomes

Required qualifications

• Current active, unencumbered compact nursing license
• Graduate of an accredited School of Nursing
• 2+ years of experience in care management or with CKD/ESRD patients
• Self-starter with the ability to work independently with minimal supervision
• Familiarity with Microsoft Office and mobile/web-based applications
