Compact RN Escalation Nurse

Job Summary

Serving as the senior clinical escalation resource for care management and remote patient monitoring, the full-time RN Escalation Nurse will assess complex patient concerns, provide clinical oversight, and coordinate care remotely across assigned shifts.

Key responsibilities

• Assess escalated patient concerns and RPM alerts to determine urgency and appropriate interventions
• Conduct direct outreach to patients for urgent clinical issues and communicate with providers to coordinate resolutions
• Provide clinical guidance to the LPN Escalation team and document all assessments and interventions in compliance with standards

Required qualifications

• Current active, compact RN license
• Prior experience in triage, telehealth, emergency department, or urgent care settings preferred
• Strong independent clinical judgment and decision-making skills in a fast-paced environment
• RN degree (ADN or BSN) from an accredited nursing program
• Willingness to work assigned shifts, including evenings or weekends as needed
