Job Summary
Serving as the senior clinical escalation resource for care management and remote patient monitoring, the full-time RN Escalation Nurse will assess complex patient concerns, provide clinical oversight, and coordinate care remotely across assigned shifts.
Key responsibilities
- Assess escalated patient concerns and RPM alerts to determine urgency and appropriate interventions
- Conduct direct outreach to patients for urgent clinical issues and communicate with providers to coordinate resolutions
- Provide clinical guidance to the LPN Escalation team and document all assessments and interventions in compliance with standards
Required qualifications
- Current active, compact RN license
- Prior experience in triage, telehealth, emergency department, or urgent care settings preferred
- Strong independent clinical judgment and decision-making skills in a fast-paced environment
- RN degree (ADN or BSN) from an accredited nursing program
- Willingness to work assigned shifts, including evenings or weekends as needed
