Compact RN Pediatric Care Manager
Job Summary
A company is looking for a Newborn/Pediatric Nurse Care Manager who is passionate about holistic member care throughout their healthcare journey.
Key Responsibilities:
- Deliver coordinated, patient-centered virtual Care Management by telephone and video to improve health outcomes
- Create impactful care plans with members and the care team to achieve desired health goals
- Assist members in navigating complex medical conditions, treatment pathways, and the healthcare system
Required Qualifications:
- Bachelor of Science in Nursing (BSN)
- Active Compact RN license in good standing
- Current CCM Certification or eligibility for CCM in August
- 5+ years of nursing experience, including 2+ years in a Newborn/Pediatric setting
- Willingness to work Monday-Friday, 9am-6pm PST
