Compact RN Telephonic Case Manager

Job Summary

Working remotely on a contract basis, the Compact RN Telephonic Case Manager will deliver telephonic medication therapy management (MTM) services, focusing on high-risk members to identify and resolve medication-related problems while ensuring optimal medication regimens.

Key responsibilities

• Perform telephonic comprehensive medication reviews and clinical assessments for targeted members
• Identify, document, and facilitate the resolution of medication-related issues
• Create written summaries and materials for prescriber offices to aid in resolving medication-related problems

Required qualifications

• Associate's Degree in Nursing and Compact Licensure in State of Practice
• Minimum of 2 years of related practice experience
• Active nurse license in a Compact Licensure State
