Compact RN Telephonic Case Manager

Job Summary

Identifying and coordinating appropriate levels of care, the full-time Telephonic Case Manager RN Medical/Oncology will manage clinical operations and medical management activities remotely, focusing on assessing, planning, implementing, and evaluating care for members.

Key responsibilities

• Make outbound and receive inbound calls to assess members' current health status
• Identify gaps or barriers in treatment plans and provide patient education for self-management
• Coordinate referrals to outside sources and ensure a complete continuum of quality care through communication with members

Required qualifications

• Active, unrestricted Compact RN License
• 3+ years of experience in a hospital, acute care, or direct care setting
• Proven ability to type and navigate a Windows-based environment
• Access to high-speed internet (DSL or Cable)
• Established dedicated work area that ensures information privacy
