Job Summary

Providing exceptional care and support for patients with chronic conditions, the remote Compact State Licensed LPN Coordinator will manage disease education, create personalized action plans, and coordinate with clinical team members to enhance patient engagement and experience.

Key responsibilities

• Provide disease management and health education to patients
• Create personalized action plans to help patients manage chronic conditions
• Coordinate with clinical team members to ensure an exceptional patient experience

Required qualifications

• Compact state license required, with additional licensing as needed
• 2+ years of experience as an LPN or LVN
• 2+ years of experience in patient-facing or customer-facing roles
• Familiarity with digital applications like Slack, Coda, and Google Workspace
• Experience in telehealth or healthcare startup environment preferred
