Job Summary

A company is looking for a RN Case Manager to provide remote clinical support and case management services to patients.

Key Responsibilities

• Conduct comprehensive telephonic assessments of patients' health status and care needs
• Develop individualized care plans in collaboration with patients and healthcare teams
• Monitor patient progress and adjust care plans to ensure continuity and quality of care

Required Qualifications

• Active RN license in a compact state (multi-state licensure)
• Minimum of 2 years of clinical experience, preferably in case management or chronic disease management
