Complex Care Outreach Coordinator

Job Summary

Working remotely full-time, the Complex Care Outreach Coordinator will conduct outreach to educate members about clinical programs, schedule appointments, and provide inbound call support while collaborating with a multidisciplinary care team to enhance patient care for complex populations.

Key responsibilities

• Conduct outreach via telephone and SMS to inform members about clinical programs and ensure they remain connected to care
• Schedule initial appointments and manage calendars to accommodate urgent care needs
• Provide inbound call support for patients with clinical or administrative questions and document member needs accurately

Required qualifications

• Bilingual in Spanish with experience engaging across various cultures
• A minimum of 3 years of healthcare experience in a customer or patient-serving role
• Willingness to work Monday to Friday from 8:30 AM to 5 PM Pacific Standard Time
