Job Summary

A company is looking for a Complex Care Physician to deliver direct virtual patient care and lead interdisciplinary teams.

Key Responsibilities

• Deliver exceptional care to complex patients using an AI-based platform and video telehealth visits
• Clinically manage patients focusing on complex geriatric conditions and monitor clinical performance metrics
• Engage with patients, caregivers, and healthcare providers to ensure cohesive care experiences and optimal outcomes

Required Qualifications

• Board Certified in Geriatrics, Primary Care, Internal Medicine, or Family Medicine
• Minimum of 2 years of clinical experience
• Experience providing clinical care in collaboration with interdisciplinary teams
• Experience working with complex, frail, or vulnerable populations
• Experience with virtual care is a plus
