Job Summary

A company is looking for a Connected Care Assistant to support members in managing their health conditions.

Key Responsibilities:
  • Engage with members through calls, chats, and outreach to facilitate care coordination
  • Assist in administrative tasks such as composing communications and managing care plan tasks
  • Attend training and comply with healthcare regulations while supporting the clinical team
Required Qualifications:
  • Prior experience in case management or healthcare setting preferred
  • Proficiency in motivational interviewing techniques
  • Ability to multitask and adapt in a dynamic environment
  • Experience with health IT systems and community healthcare resources is desirable
  • Commitment to a schedule of 8 AM to 5 PM (PST/CT/EST) Monday through Friday
