Job Summary

A company is looking for a Connected Care Coach responsible for driving behavior changes and positively impacting clinical outcomes.

Key Responsibilities
  • Conduct member outreach to promote behavior change and improve clinical outcomes through various communication tools
  • Utilize a coaching approach to empower members in self-managing their chronic conditions
  • Maintain accurate case documentation and complete essential administrative duties
Required Qualifications
  • Bachelor's Degree in a relevant discipline or MA/EMT/Paramedic certification
  • 2+ years of experience in a healthcare delivery or related environment
  • Ability to work the specified schedule of 1:45pm-10:15pm CST
  • Experience with multiple technology platforms including MS Office and Salesforce
  • Bilingual in Spanish and English is preferred
