Job Summary
A company is looking for a Connected Care Coach responsible for driving behavior changes and positively impacting clinical outcomes.
Key Responsibilities
- Conduct member outreach to promote behavior change and improve clinical outcomes through various communication tools
- Utilize a coaching approach to empower members in self-managing their chronic conditions
- Maintain accurate case documentation and complete essential administrative duties
Required Qualifications
- Bachelor's Degree in a relevant discipline or MA/EMT/Paramedic certification
- 2+ years of experience in a healthcare delivery or related environment
- Ability to work the specified schedule of 1:45pm-10:15pm CST
- Experience with multiple technology platforms including MS Office and Salesforce
- Bilingual in Spanish and English is preferred
