Job Summary

A company is looking for a Connected Care Coach to drive behavior changes and positively impact clinical outcomes for members.

Key Responsibilities

• Conduct member outreach to promote behavior change and improve clinical outcomes
• Utilize a coaching approach to empower members in self-managing chronic conditions
• Maintain accurate case documentation and complete essential administrative duties

Required Qualifications

• Bachelor's Degree in a relevant discipline or MA/EMT/Paramedic certification
• 2+ years of experience in a healthcare delivery or related environment
• Ability to work 1:45pm-10:15pm CST
• Flexible and able to work in a fast-paced environment
• Bilingual in Spanish and English is preferred
