Connecticut Licensed Care Manager
Job Summary
A company is looking for a Care Manager (RN).
Key Responsibilities
- Evaluate member needs and recommend care plans to address barriers to accessing appropriate care
- Develop ongoing care/service plans and collaborate with providers to meet members' unmet needs
- Provide education and resource support to members and their families regarding healthcare options and benefits
Required Qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Registered Nurse (RN) state licensure or compact state licensure required
- Bilingual in Spanish and English preferred
- Must reside in Connecticut
