Connecticut Licensed Care Manager

Job Summary

A company is looking for a Care Manager - Virtual Cardiac Rehab (Contract).

Key Responsibilities

• Guide participants through a 3-month online program via video calls and group sessions to manage cardiac-related risk factors
• Develop and modify comprehensive treatment plans tailored to individual patient needs, including exercise, medications, and lifestyle interventions
• Engage patients weekly for coaching and support, and coordinate treatment plan approvals with Medical Directors

Required Qualifications

• Minimum Bachelor's degree in exercise-related science or nursing
• 1-2 years of experience in telehealth, remote patient monitoring, chronic care management, or virtual care
• Certifications: ACSM-CEP for Exercise Physiologists or CCRP for Registered Nurses (or willingness to obtain within 6 months)
• 1-5 years of experience in cardiac education, care management, cardiac rehab, and secondary prevention
• Ability to work 15-20 hours/week with a commitment to a set schedule for a minimum of 3 months
