Job Summary

A company is looking for a Case Manager Registered Nurse.

Key Responsibilities

• Engage with members telephonically to coordinate their care
• Develop and implement individualized care plans based on comprehensive assessments
• Document care management activities accurately in the electronic health record

Required Qualifications

• Active and unrestricted Registered Nurse (RN) licensure in Connecticut or compact licensure in state of residence
• Proficient in Microsoft Office Suite
• Minimum of 3+ years of nursing experience
• Minimum of 2+ years of case management or home healthcare coordination experience
• Access to a private space for remote work
