Required Qualifications

• Active and unrestricted Registered Nurse (RN) licensure in Connecticut or compact licensure
• Proficient in Microsoft Office Suite
• Minimum 3+ years of nursing experience
• Minimum 2+ years of case management or home healthcare coordination experience
• Access to a private workspace for remote work
