Job Summary

A company is looking for a CT Licensed Clinician: LMSW or LCSW in Behavioral Health.

Key Responsibilities

• Provide customized care and treatment plans for patients within assigned Care Pods
• Coordinate behavioral health care with primary care providers and monitor patient progress
• Document patient interactions and treatment recommendations in electronic health records

Required Qualifications, Training, and Education

• Master's degree in Social Work, Counseling, or a related field from an accredited program
• Licensed to practice in Connecticut as an LMSW or LCSW
• Experience with evidence-based treatment approaches
• Technical proficiency in electronic health records and various platforms
• Knowledge of behavioral health screening tools
