Job Summary

A company is looking for a CT Licensed Clinician: LMSW or LCSW in Behavioral Health.

Key Responsibilities

• Provides customized care and treatment plans for patients within assigned Care Pods
• Coordinates behavioral health care with primary care providers and monitors patient symptoms and treatment effectiveness
• Documents patient progress and treatment recommendations in the electronic health record and participates in supervision and consultation sessions

Required Qualifications, Training, and Education

• Master's degree in Social Work, Counseling, or a related field from an accredited program
• Licensed to practice in Connecticut as an LMSW or LCSW
• Secure, high-speed internet connection capable of supporting video conferencing
