Connecticut Licensed Neurology Nurse Practitioner

Job Summary

Seeking a skilled and compassionate Connecticut Licensed Neurology Nurse Practitioner, the full-time remote position will provide direct clinical care to patients with neurological conditions via telehealth, while collaborating with physicians and the care team to enhance patient access and treatment efficiency.

Key responsibilities

• Provide direct clinical care to patients with neurological conditions and memory disorders through telehealth
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians
• Educate patients and families on diagnoses, treatment plans, and disease progression while maintaining accurate clinical documentation

Required qualifications

• Active, unrestricted Nurse Practitioner license in Connecticut; certification in geriatrics, adult-gerontology, or family practice preferred
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
• Strong clinical judgment and clinical documentation skills
• Ability to manage a full patient panel independently with physician collaboration
• Excellent communication skills with patients facing cognitive decline and their caregivers
