Connecticut Licensed Nurse Practitioner

Job Summary

To enhance patient care in a virtual setting, the full-time Connecticut Licensed Nurse Practitioner will provide direct clinical care for patients with neurological conditions and memory disorders, collaborating closely with physicians and the care team while working remotely from Connecticut.

Key responsibilities

• Provide telehealth clinical care to patients with neurological conditions and memory disorders
• Conduct patient assessments, develop care plans, and manage treatment in collaboration with supervising physicians
• Educate patients and families on diagnoses, treatment plans, and disease progression

Required qualifications

• Active, unrestricted NP license in Connecticut; certification in geriatrics, adult-gerontology, or family practice preferred
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
• Strong clinical judgment and documentation skills
• Ability to manage a full patient panel independently with physician collaboration
• Excellent communication skills with patients facing cognitive decline and their caregivers
