Job Summary
To support a growing Population Health team, a Connecticut Licensed Nurse Practitioner will provide telephonic and video-based follow-up care for patients post-hospital discharge, focusing on medication reconciliation, symptom screening, and care coordination, while working a minimum of 24 hours per week in a flexible, remote environment.
Key responsibilities
- Manage telephonic outreach and scheduled video/phone visits for recently discharged patients
- Conduct medication reconciliation, symptom screening, and order necessary follow-up labs or medications
- Coordinate care across various settings, ensuring continuity and connection for patients
Required qualifications
- Certified Nurse Practitioner in Adult, Family, or Geriatrics
- Active state licensure in Connecticut, with the ability to obtain additional state licenses
- 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
- Experience engaging Medicare and Medicaid populations managing complex conditions
- Comfort with digital tools and virtual care delivery
