Job Summary

A company is looking for a Connecticut Licensed Occupational Therapist (Care Navigator) - REMOTE.

Key Responsibilities

• Provide remote monitoring services to support patients in their home-exercise therapy program
• Direct and assist users in therapeutic exercise, neuromuscular re-education, and functional training through the technology platform
• Maintain necessary records and provide ongoing feedback to the evaluating clinician on patient progress

Required Qualifications

• Active Connecticut OT license
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy with experience in a multi-platform environment
• Experience in behavioral modification and Health Coach Certification is a plus
