Job Summary

A company is looking for a Connecticut Licensed Physical Therapist Assistant (Care Navigator) - REMOTE.

Key Responsibilities

• Provide remote monitoring services to support patients in their home-exercise therapy program
• Direct and assist users in therapeutic exercise and functional training through a technology platform
• Maintain records and provide ongoing feedback to clinicians on patient progress

Required Qualifications

• Active Connecticut PTA license
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy, comfortable with multiple platforms (EHR, messaging, scheduling)
• Experience in behavioral modification or Health Coach Certification is a plus
