Job Summary
To provide telephonic care and coordination for patients, the part-time Connecticut Licensed Telephonic Nurse Practitioner will manage calls and deliver quality care remotely, ensuring compliance with documentation and care standards while covering various shifts, including evenings, nights, and weekends.
Key responsibilities:
- Provide telephonic and virtual care, coordinating with caregivers, facilities, and healthcare providers
- Utilize electronic medical records to deliver acute care and document findings in compliance with regulations
- Coordinate care transitions and monitor patient needs, adjusting care plans as necessary
Required qualifications:
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner license in Connecticut
- Board certification as a Nurse Practitioner through a national board
- Current DEA licensure/prescriptive authority or ability to obtain post-hire
- Proficient in using electronic medical records for documentation
