Job Summary

Consultative Pharmacist - PA/TN Licensed, full-time remote position responsible for providing clinical review and consultation on medication requests, evaluating therapy appropriateness, and collaborating with providers to enhance patient care.

Key Responsibilities

• Provide clinical review and consultation on medication requests and prior authorizations
• Evaluate therapy appropriateness, dosing, and safety based on evidence-based guidelines
• Collaborate with providers to clarify prescriptions and recommend alternatives

Required Qualifications

• R.Ph required; PharmD preferred
• Full, current, and unrestricted pharmacist license in PA or TN
• 1+ year of pharmacy experience preferred (retail, hospital, or managed care)
• Working knowledge of formulary management and clinical guidelines
• Commitment to evidence-based practice and continuous learning
