Consultative Pharmacist - TN/PA Licensed

Job Summary

Providing clinical review and consultation on medication requests, the full-time Consultative Pharmacist - TN/PA Licensed will work remotely to evaluate therapy appropriateness, collaborate with providers, and support complex case reviews while ensuring cost-effective patient care.

Key responsibilities

• Provide clinical review and consultation on medication requests, prior authorizations, and benefit determinations
• Evaluate therapy appropriateness, dosing, and safety based on evidence-based guidelines
• Identify opportunities to improve medication adherence, safety, and cost-effectiveness

Required qualifications

• R.Ph required; PharmD preferred
• Full, current, and unrestricted pharmacist license in PA, TN, and/or OH
• 1+ year of pharmacy experience preferred (retail, hospital, or managed care)
• Working knowledge of formulary management, clinical guidelines, and pharmacy benefit structures
• Commitment to evidence-based practice and continuous learning
