Consultative Pharmacist - TN/PA Licensed

Job Summary

Providing clinical consultation and medication management, the full-time Consultative Pharmacist - TN/PA Licensed will work remotely to evaluate therapy appropriateness, collaborate with providers, and support complex case reviews while ensuring cost-effective patient care.

Key responsibilities

• Provide clinical review and consultation on medication requests and prior authorizations
• Evaluate therapy appropriateness and dosing based on evidence-based guidelines
• Collaborate with providers to clarify prescriptions and recommend alternatives

Required qualifications

• R.Ph required; PharmD preferred
• Full, current, and unrestricted pharmacist license in PA, TN, and/or OH
• 1+ year of pharmacy experience preferred (retail, hospital, or managed care)
• Working knowledge of formulary management and clinical guidelines
• Commitment to evidence-based practice and continuous learning
