Consultative Pharmacist - TN/PA Licensed

Job Summary

Providing clinical review and consultation on medication requests, the full-time Consultative Pharmacist - TN/PA Licensed will work remotely to evaluate therapy appropriateness and collaborate with providers to enhance patient care.

Key responsibilities

• Conduct clinical reviews and consultations on medication requests and prior authorizations
• Evaluate therapy appropriateness, dosing, and safety based on evidence-based guidelines
• Educate internal teams on formulary management and pharmacy policy updates

Required qualifications

• R.Ph required; PharmD preferred
• Full, current, and unrestricted pharmacist license in PA, TN, and/or OH
• Ability to obtain additional state licensure based on business needs
• 1+ year of pharmacy experience preferred (retail, hospital, or managed care)
• Working knowledge of formulary management and clinical guidelines
