Job Summary

A company is looking for a Continuing Care Specialist.

Key Responsibilities

• Guide clients and families in recognizing the value of continuing care and assist in qualifying opportunities
• Manage client pipelines and develop continuing care plans while achieving conversion and transfer goals
• Communicate with stakeholders and ensure a smooth continuation of care process for clients

Required Qualifications

• At least one year of experience in the behavioral health industry in a case manager or outreach/intake role
• At least one year of experience in a sales capacity
• Preferred Bachelor's degree in mental health or a related field
• Technology savvy with experience in video technology and telehealth platforms
• Ability to pass a state background check
