Job Summary

A company is looking for a Crisis Care Manager.

Key Responsibilities

• Provide emotional support and care coordination for families of behaviorally and medically complex children
• Assess case referrals, identify resource needs, and monitor case progress for closure
• Connect families with community resources and support groups to aid in recovery goals

Required Qualifications

• Master's degree in Social Work (MSW) required
• Bilingual in English/Spanish preferred
• 2+ years of case management or relevant experience
• Experience in child & adolescent case management and acute behavioral health needs
• Strong knowledge of community resources
