Job Summary
To support community health initiatives, the full-time Population Health Specialist will conduct proactive outreach, assist patients in accessing healthcare services, and utilize data to monitor progress and improve health outcomes in a fully remote role available to Missouri residents only.
Key Responsibilities
- Conduct proactive outreach to patients and individuals who may benefit from Health Center services
- Identify and address care gaps through outreach, education, scheduling, and follow-up
- Use patient and program data to track outcomes and support quality improvement initiatives
Required Qualifications
- High School/GED required; Bachelor's degree preferred
- At least three (3) years' experience required
- Proficiency in Microsoft Office applications, including Word, Excel, and Outlook
- Must obtain and maintain certification in an approved crisis intervention methodology within 60 days of hire
