Key Responsibilities

Provides enrollment support for new PACE enrollees
Communicates pharmacy benefit information to network pharmacies
Coordinates facility discharges to ensure medication reconciliation and delivery

Required Qualifications

High School diploma required
Minimum of 1 year customer service experience, preferably in a healthcare setting
