Delaware Licensed Memory Care Specialist

Job Summary

To enhance care for patients with memory disorders, the full-time remote Delaware Licensed Memory Care Specialist will manage virtual patient care, clinical triage, and caregiver support while collaborating with an interdisciplinary team.

Key Responsibilities

• Manage patient, caregiver, and provider communications through Epic In Basket
• Evaluate and triage urgent clinical concerns related to memory disorders
• Conduct virtual visits and provide follow-up care between scheduled appointments

Required Qualifications

• Master's or Doctoral degree from an accredited Nurse Practitioner or Physician Assistant program
• Board Certification as an FNP, AGNP, AGPCNP, PA-C, or PMHNP with relevant experience
• Current Delaware APRN or PA license, or eligibility for Delaware licensure
• DEA registration and Delaware Controlled Substance Registration (CSR)
• Fellowship training or a minimum of three years' clinical experience in geriatrics or related specialty
